STEP 1
If you are experiencing difficulties that you think may be due to ADHD, book and attend an appointment with your GP to discuss your problems and how those affect your daily life in detail.

Below is a letter that you may personalise and address to your GP before or during the appointment with your GP.

STEP 2
If your GP thinks an assessment is appropriate, ask to be referred to ADHDNET under Right to Choose Framework.

STEP 3
Ask your GP to complete and email our referral form and your summary care record, we will acknowledge in writing that the referral is received within 1 working day.

STEP 4
We will contact you to arrange a suitable date within 10 working days.

STEP 5
We will ask you to complete our preassessment forms prior to the assessment.

STEP 6
We will complete your assessment within 8 weeks from date of the receipt of the referral.

STEP 7
We will share the assessment report within 4 weeks from completion of the assessment.

STEP 8
If you are diagnosed and medication indicated, you will be offered medication titration within 8 weeks of completion of assessment.

STEP 9
Titration will be concluded within three rounds of final chosen medication.

STEP 10
We will engage in shared care with your GP to ensure continuation of treatment.

Step 11
We will address any concerns and queries from you and your GP during shared care.

STEP 12
We will review your care and treatment annual.
